OASIS-D: Physical Therapy Discharge Assessment

UPDATED for OASIS-D and compliant with the CoPs -  this 22-page booklet is used to document updates to the comprehensive assessment.This assessment integrates the OASIS data sets for discharge when patient isreturning to the community (not aninpatient facility); as well as the acceptable reasons for transfer ordischarge of the patient from the HHA as noted in 484.50(d). 

In addition, space for the narrative summary has been expanded to allow for theincreased scope of information required at either time of transfer or dischargedue death at home.
