the pediatric patient s comprehensive assessment findings and then initiate the
Briggs Care Plan. This head-to-toe assessment prompts the nurse to identify
problems relevant to the urgent need for nursing intervention.
This assessment was updated to meet the requirements of the 2018 home care
conditions of participation (CoPs) collecting both the subjective and objective
assessment data. Data collection includes a brief history, current health,
functional, cognitive status, strengths/limitations and psychosocial aspects.
Consistent with the principle of patient centered care the form addresses the
personal goals, care preferences and the need for patient,
caregiver/representative/family education and training.
Advanced discharge planning, identification of risk factors for hospital
admission and/or emergency room care are just some of the other features
included.
